Accidents and incidents may require investigating to prevent reoccurrences. Initially a witness, a Public Safety Officer or the injured person will provide details of the event and any contributing factors. Any action taken at the time of the event to render a condition safe should be noted. The Director of EHS may investigate further to consider the necessary actions required to prevent a reoccurrence or removal of the hazard.

The dictionary definition of an accident as “an unforeseen event” is not particularly helpful in the context of health & safety. The following definitions describe events from a health and safety perspective:

• Accident – Any unplanned occurrence that leads to the injury of any person involved in activities; including diseases or medical conditions.

• Incident – Any unplanned occurrence that leads to property, plant or equipment damage. Any situation having the potential to cause injury or damage, but not actually doing so. A near miss.
